This form is intended for Canadian Registered Charities seeking permission to collect Zakat at the Islamic Society of Markham (ISM) during the approved Zakat Day(s). Please note the following important guidelines: Submission of this form does not guarantee approval. All approved organizations must bring: a) Three display banners b) At least four (4) credit card machines c) A minimum of four (4) male volunteers and one (1) female volunteer (more are encouraged) Please ensure you complete the form accurately and thoroughly. Applications will be reviewed internally, and you will be contacted if your request is approved.
